In order to gain entry into AOD/COL system, please complete and submit the following forms and return them to our office for processing:
- Property Data Sheet
- Building/Unit Set-Up Form
- Authorized Representative Designation
- AOD/COL Agreement
Once the above noted forms have been submitted, MHC will provide a current Tenant Recertification Report, per development, for review of accuracy. Upon MHC's receipt and update, if needed, you will be assigned an initial Username and Password to gain entry into the AOD/COL system.
